§ 111.165. License periods—Fees—Transferability.

    License Periods.

    1.

    Food establishment operations licenses shall be issued in accordance with state law.

    2.

    Temporary food establishment operations licenses shall be issued only for the number of days, not to exceed five, designated on the temporary food establishment operation application.

    B.

    Fees.

    1.

    Food establishment operations license fees shall be as follows:

    a. One checkout $ 50.00
    b. Two to four checkouts $100.00
    c. Five or more checkouts $150.00
    d. Temporary food establishment operation $ 5.00
    e. Non-commercial food establishment $ 15.00
    f. Mobile food establishment $100.00

     

    2.

    If a new or renewal food establishment operations license fee is received by the Health Department after January 31, 2001, the operator will be assessed a late fee equal to twenty-five percent of the fee due.

    C.

    Transferability.

    1.

    No food establishment operation license is transferable to the purchaser or lessor of that food establishment operation without verification to the Health Department that the purchaser or lessor acquired more than fifty percent of the value of the food establishment operation.

    2.

    No temporary food establishment operation license is transferable.
