Article 1. Ambulances and Emergency Medical Services* |
§ 111.005. Renewal.
A.
Registration issued pursuant to this chapter shall expire on December 31st of each year and may be renewed by the Licensing Agent, if all of the following apply:
1.
The applicant requesting renewal completes and submits the renewal application to the Licensing Agent at least ten days, but no more than thirty days, prior to the registration expiration date of December 31st;
2.
The applicant evidences that it is licensed and in good standing with the Ohio Ambulance Licensing Board;
3.
The applicant remits the fifty-dollar renewal fee;
4.
The applicant executes the consent form authorizing random inspections by the Ambulance Inspector of any aspect of the applicant's organization or services, including, but not limited to, premises, ambulances, validity of certifications at the business or its EMT-Bs, EMT-Is, EMT-Ps or its ambulances; and
5.
The applicant executes the notarized statement required by § 111.004(B)(14) and (B)(15).
