Completed application forms shall be accompanied by a nonrefundable annual license fee of twenty-five dollars for a new license and fifteen dollars for a license renewal. 5.

Other information required by the Alarm Administrator necessary for the enforcement of this article.

    D.

Any false statement of a material matter made by an applicant for the purpose of obtaining a license shall be sufficient cause for refusal to issue a license or to revoke a license.
