§ 90.09. Forms.

A.

The Department of Public Health shall make available to physicians performing abortions in the City constructive notice forms, consent forms, abortion report forms, and complication report forms as provided for in §§ 90.04, 90.05, and 90.08.

B.

The Department of Public Health shall make available to all physicians, who may perform post-abortion care, complication report forms as provided for in § 90.08.
